 The Claim Form and panel of Appointed Assessors for both ElderShield and MyCare are the same. You only need to complete the Claim Form once.
If you are currently unable to perform three or more activities of daily living (washing, dressing, feeding, toileting, transferring and mobility), please follow the three simple steps below to make a claim.

Print a copy of the Claim Form and list of Appointed Assessors*
Alternatively, you may obtain copies from our Customer Service Centre at: 4 Shenton Way SGX Centre 2 #01-01, Singapore 068807.

Complete the Claim Form as best you can
If you are unable to do so, please ask an immediate family member or caregiver to complete it for you. If you still need help, you can contact our Customer Service Hotline on 6827 7788.

Make an appointment with an Appointed Assessor for your severe disability assessment
You will have to:
 |
Pay $25 if the assessment is done at the Assessor’s clinic |
 |
Pay $100 if a house call is required |
 |
Bring the completed Claim Form and any available medical records and/or Inpatient Discharge Summary that you have |
 |
Bring your medicine (if possible). |

The Assessor will send the completed Claim Form together with the results of the severe disability assessment to your insurer for processing.
